Yellow Form Driver's Medical Report

Service Description

Iridia will soon be offering Class 1, Class 2, Class 3, and Class 4 (Restricted and Unrestricted) drivers in need of a Driver’s Medical Examination Report can now come to the IMCC to complete their DMER. The Yellow Form DMER is used by RoadSafetyBC to ensure there is an acceptable level of driver medical fitness. The DMER checks for: • Cognition • Overall Physical Health • Eyesight This service is for age related and if deemed necessary for commercial reassessment for Class 1, Class 2, Class 3, and Class 4 drivers. The $225 cost is inclusive of all testing mandatory to satisfy the DMER requirements, booking fees, and taxes. Please Note: Tests are run in our clinic and results are typically processed within 20 minutes. We only perform testing for asymptomatic individuals. Therefore, if you are experiencing symptoms of COVID-19 you should follow the guidelines of local Public Health Authorities. Cancelation Policy

If you need to cancel or reschedule your appointment, please do so a minimum of 72 hours in advance. A full refund will be issued if you cancel with a minimum of 72 hours. A refund will take 7-10 business days. Please note that appointments cancelled within 72 hours of the appointment as well as missed appointments, will only receive a partial refund and are subject to a fee of $50.
